All medical services, patient intake, billing, subscriptions, and communications are delivered through a third-party telehealth platform (Practice Better). Our website is provided for informational and administrative purposes only.

2. No Emergency Services

Our services are not intended for emergency or urgent medical care.

If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. Do not use this website or telehealth services for emergency situations.

3. Medical Disclaimer & Provider Discretion

  • All medical services are provided by licensed healthcare professionals.

  • Medical decisions are made on an individual basis.

  • Providers may decline to prescribe medications or provide treatment if they determine it is not medically appropriate or safe.

  • Participation in telehealth services does not guarantee approval for treatment or prescriptions.

4. Eligibility & Use of Services

To use our services, you must:

  • Be at least 18 years of age

  • Be legally capable of entering into a binding agreement

  • Provide accurate and truthful information when requested

Telehealth services are provided only where permitted by applicable law and provider licensure requirements.

5. Telehealth Platform & Third-Party Services

Midwest 360 Wellness uses Practice Better to deliver telehealth services, including:

  • Scheduling

  • Medical intake

  • Consultations

  • Payments and subscriptions

  • Secure communications

We are not responsible for downtime, technical failures, or service interruptions related to third-party platforms. Delays or disruptions caused by technology issues do not constitute a breach of these Terms.

You are responsible for maintaining reliable internet access and compatible devices.

6. Payments, Subscriptions & Billing

All payments are processed through our telehealth platform.

Important billing terms:

  • Consultation fees are non-refundable

  • Subscription fees are non-refundable once initiated

  • Subscriptions may be canceled with at least 7 days’ notice

  • Medications are non-refundable once prescribed, processed, or shipped

  • Missed appointments or failure to complete required steps does not entitle you to a refund

Pricing, billing cycles, and payment terms may be updated from time to time and will be communicated through the platform.

7. Results Disclaimer

  • Individual results vary based on medical history, adherence, and other factors

  • Testimonials or success stories represent individual experiences and are not typical results

  • Outcomes differ from person to person

8. Communications Consent

By using our services, you consent to receive:

  • Service-related emails

  • Appointment reminders

  • Administrative and operational notifications

We do not send sensitive medical information through unsecured communications.

9. User Conduct

  • Refuse service at provider discretion

  • Discontinue services if continued participation is deemed unsafe or inappropriate

Termination does not relieve you of outstanding payment obligations.

11. Limitation of Liability

To the fullest extent permitted by law:

  • Midwest 360 Wellness is not liable for indirect, incidental, or consequential damages

  • Any claim arising from use of our services is limited to the amount you paid for services in the preceding 12 months

12. Indemnification

You agree to indemnify and hold harmless Midwest 360 Wellness & Rejuvenation LLC, its providers, employees, and partners from any claims arising from your misuse of services or violation of these Terms.

13. Governing Law & Dispute Resolution

These Terms are governed by the laws of the State of Wisconsin, without regard to conflict of law principles.

Any disputes arising under these Terms shall be resolved exclusively in the state or federal courts located in Wisconsin, and you consent to their jurisdiction.

14. Changes to These Terms

We may update these Terms from time to time. Updates will be posted on this page with a revised effective date. Continued use of services constitutes acceptance of the updated Terms.
